One of the leading concerns among modern times for women is the inability to conceive. Stressful lives with busy work schedule, meetings & deadlines leaving no time for exercise; junk and fast food replacing healthy home cooked food; alcohol, smoking and recreational drugs during weekend chill out to unwind and destress – lifestyles that are a perfect cocktail for infertility. Sometimes the cause can be biological – genetic, hormonal – PCOS, ovarian failure, ovulation, fibroids uterus, thin endometrium or anatomical – tubal disease, endometriosis and many more.
One such factor that leads to complications with conception is an important process of Ovulation. Ovulation refers to the process of release of eggs from the ovaries. The ovarian follicles rupture and the secondary oocyte ovarian cells are released, which are then available to be fertilized by the sperm. But at times, conditions such as Oligo-ovulation – that is infrequent ovulation or Anovulation – where ovulation does not occur at all i.e, follicles do not mature can lead to infertility. As we comprehend, it is easier to treat causes that are external to the body while those internal need to be understood so that the exact cause can be treated effectively.
Luckily, modern science and technology have discovered a treatment even for conditions such as irregular ovulation or lack of ovulation by inducing ovulation. Hence the name Ovulation Induction. It is one of the kinds of treatments where eggs are made to mature and be released from the ovaries for fertilization, to match times intercourse or artificial insemination. Here is how it works.
- Assessment – Prior to the procedure, assessments are carried out. Levels of various types of hormones such as thyroid, prolactin, Follicle Stimulating Hormone (FSH). Luteinizing hormone (LH), testosterone and other male androgens are assessed through blood tests. The ovulation cycle is tracked. Internal organs such as the womb, ovaries, follicles and the thickness of the uterine lining are assessed with the help of Ultrasound.
- Stimulation – Once the assessments are made, the next most important step of Stimulation is carried out. In this stage, medications are prescribed to help the eggs in the follicles to mature and to be released. Medications such as Clomiphene Citrate are administered orally to trigger the release of hormones such as FSH and LH that aid in ovulation. Other alternatives such as Letrozole, Gonadotrophins and Gonadotrophin-releasing hormones amalogs are also often employed, even during IVF, to induce maturation of the follicles.
- Monitoring – Goes hand in hand throughout the process. Monitoring the growth of the number of follicles and the size of the follicle is essential
- Timed Intercourse or Artificial Insemination – Once the adequate number of follicles mature and the eggs are released, the couple is advised to have intercourse, which is often called ‘Timed Intercourse’, as the chances of the eggs getting fertilized are higher at that particular point in time. At times, Intra-uterine Insemination is also performed, where the semen collected is inserted into the uterus for the eggs to be fertilized.
Thus, the procedure helps in initiating the process of ovulation and release of an adequate number of eggs. The process is also often recommended in cases of IUI, IVF or unexplained infertility. The success rate is up to 20% and is one of the least invasive methods of treatment.
